Program description
The Primary Care Access Program (PCAP) is a low-cost coverage program that provides primary health care services to adults living in Santa Clara County who do not have access to health insurance. Enrollees select a community clinic and receive primary and preventive care services there.
Eligibility:
- Adults ages 19 and older who live in Santa Clara County;
- Household income at 650%* or less of the Federal Poverty Level; and
- Adults who do not have access to health insurance including employer insurance, Medicare, full scope Medi-Cal, Covered California, or IFP insurance.
*Effective July 1, 2023
Patients may enroll by calling or visiting a participating Community Clinic to make an appointment with an Enrollment Counselor or ask the front desk staff for more information. PCAP is administrated by Valley Health Plan (VHP), which is part of the County of Santa Clara.
