Reduce your risk for developing type 2 diabetes through a 16-week prevention program. The program is designed for members 18+ years of age, at risk of type 2 diabetes due to family history, women who have had gestational diabetes during pregnancy, and lifestyle habits. You will learn how to eat healthy, increase your physical activity, and lower your blood sugar.

If you are struggling with obesity and are looking for help, consult your doctor, then schedule your bariatric orientation. You will learn about the different styles of weight loss surgeries, bariatric support groups, and steps to qualify for bariatric surgery.
Santa Clara Family Health Plan members must attend this orientation in order to be eligible to have Bariatric Surgery.

This one-week camp is offered only during summer to members ages 9 to 14 who are at risk for type 2 diabetes. The program offers fun activities while highlighting the importance of nutrition, physical activity, self-esteem, and diabetes prevention through positive lifestyle changes.

This one-week camp is offered only during summer to members ages 6 to 12 who are living with asthma. Youth will learn how to manage their asthma, including tips for recognizing triggers, all while having fun and meeting new friends.
